Greenwood Start of School Packet
Hello, Greenwood Families –
Welcome to the 25-26 school year! Please complete and return all required forms as soon as possible. If you need paper copies of any item, please email Alice Lessard at arlessard@seattleschools.org. Please provide your student’s name and which forms you would like.

DO ONLINE:
- Data Verification Form The online Student Verification Form will be available via the Source from September 3-30. Here is more information on The Source if you are setting up a new account or logging into an established account. Once in The Source, click Data Verification Form; review/update before September 30th. The system allows for only one data verification form per student. Two parents cannot submit a form for the same student.
- Family Educational Rights and Privacy Act Form (FERPA) (forms become an automatic OPT-IN on Oct. 1.)
- iPad and Laptop Agreement Form
ADDITIONAL FORMS AND INFORMATION (Submit forms in bold below, if applicable)
- Student Housing Questionnaire. Complete if you are unsheltered or in temporary housing.
- 504-2 Form. Complete if you think your child has a disability.
- Office of Indian Education Title VII Student Eligibility Certification. Complete if your student is new to SPS and you want him/her counted under the Indian Education Program.
- Free and Reduced Student Meals culinary-services/free-and-reduced-price-meals/Complete if your student is eligible, even if you don’t plan to take lunch at school – this helps school funding.
- 25-26 Protection of Pupil Rights Amendment (PPRA) Notification of Rights
School year dates, attendance information, guidance on communication tools, procedures for discrimination complaints and sexual harassment, instructions for how to get approved to volunteer in school can be found on the district Start of School Forms webpage. Translations in the most common languages are available for many of the forms.
